Typically it is easier to cast between swings of a 2h without losing melee dmg. I personally use a slow 2 hander. Disease dot, slow, root. Joust so that you only get hit by the kick/bash. Run in, melee until mob is queued to attack, run out, dog gets hit, run back in.
